I understand the
Salmon Camp programs are continuously evaluated for
the purpose of improving the programs. Some of the
ways OMSI does this is by collecting information
from participants through written questionnaires,
interviews, field notes, or other feedback methods.
Some information collected may identify you (such
as, name or email). However, your name will not be
associated with any information in evaluation
reports or publications. Original copies of data
collected will be kept in a secure location at OMSI
and only designated project staff will have access
to the information.
The risk to participants during evaluation
activities is minimal; participants may feel
uncomfortable sharing their opinions, experience
anxiety during assessment testing, be surprised at
learning about their skills or abilities, or feel
that their experience has been compromised by the
occurrence of evaluation during the program. The
benefits of participation in evaluation activities
have been documenting that SCRT is successful in
addressing students’ confidence/fluency with science
and information technology, preparing students for
careers/internships and workplace skills, and
overall success in school, and offering feedback
that continually improves the programming.
I understand that I have the option to withdraw from
any particular evaluation activity if I choose
without penalty.
